In order to apply for AWDI Certification Materials on-line, you must fill in the information below. Fields in RED Required. Once it is filled in, please fax it to us at 561-776-9827 with your payment (credit card) details. We will immediately send priority mail your manual, your set of standards and your test. Return the test, and we will grade and send you your Certificate, Membership Card, embroidered patch and list you on the site. All details will be in your transmittal.
